OKLAHOMA PURPLE HEART LICENSE PLATES
COST: $10 initial and $9 annual renewal costs for PH plate on registered vehicles and motorcycles. Personalization and handicapped symbol not available.
Military Multi-Decoration License Plate. Oklahoma also has a unique Military Multi-Decoration License Plate where you can display any three of the following six medals: Silver Star, Bronze Star, Purple Heart, Distinguished Flying Cross, Distinguished Service Award, and Ex-POW. Initial cost of this plate is $22, with $21 annual renewal fee. And on this plate, personalization is permitted at no extra cost.
PROCEDURE: in person: GO to your local Tag Agent Office with proof of registration/insurance and PH award (copy of DD-214 or other), COMPLETE Form 751-F, Application for Military Honors License Plate, and PAY applicable fees. Tag Agent will issue plate or order it to be mailed.
If you’re applying for the multi-Decoration License plate, COMPLETE Form 751-C, Military Multi-Medal License Plate, and ensure that your DD-214 or other documentation supports award of the three medals chosen for display on the plate. Tag Agent will issue or order plate for you.
By mail:
a. GO to www.tax.ok.gov/mvforms/751-F.pdf and PRINT Form 751-F, Application for Military Honors License Plate.
b. COMPLETE the form, ENCLOSE proof of PH award (copy of DD-214 or other) and payment, and MAIL to Motor Vehicle Division, Special Plate Section, 2501 Lincoln Boulevard, Oklahoma City OK 73194-0201. MVD will mail plates
c. If you’re interested in the Military Multi-Decoration License Plate, then go to www.tax.ok.gov/mvforms/751-C.pdf and PRINT Form 751-C, Military Multi-Decoration License Plate Application.
d. COMPLETE the form (note there’s a place for personalization), ENCLOSE proof of award for the three medals chosen to be on the plate (DD-214 or other) and payment, and MAIL to the same address. MVD will mail plates.
SURVIVING SPOUSE: Can use the PH plate by paying annual renewal fee, but must turn in plates if he or she re-marries. Cannot use the Multi-Decoration plate and must turn it in upon sponsor’s death and re-register vehicle for regular tags.
