Did you know if you already have a claim pending for compensation and/or pension with the VA and need to provide information or upload attachments, you can use VA Form 21-4138, Statement in Support of Claim, available in VONAPP.  When uploading attachments, include a statement that you are attaching evidence to support your claim.  After completing the form, you will have the opportunity to attach your documents.
You can attach up to 5 electronic files to any VONAPP application before you submit it using VONAPP.  The total size of all attachments must be 1 megabyte or less and must be one of the following file

types: .doc, .xls , .gif, .jpeg, .jpg, .bmp, .tif , .txt , .pdf .

The information below lists things you’ll need to prepare for filling out your application and evidence you’ll need to mail separately as supporting documentation.

Information to Prepare Answers Evidence to Send VA
1. Active Duty, Reserve Duty or National Guard Duty information
• dates and places you entered and left duty
• mailing addresses of units you served in
2. List of military benefits you receive and amounts Your DD214 or other separation papers for all periods of service unless you are filing a Benefits Delivery at Discharge or Quick Start claim

while on active duty or full-time duty.

• name and address of the medical facility
4.  Information about any exposures that caused the disabilities you are claiming, including dates that you were exposed Copies of medical records you have
Medical evidence showing you currently have this disability

If you are filing a Benefits Delivery at Discharge or Quick Start claim, you must send a copy, or original, of your service treatment records to the specified Regional Office to process your claim.  Please

call toll-free 1-800-827-1000 for the address to send your service treatment records.
5.  Information about your current spouse, including his/her Social Security number (and VA file number if he/she is a veteran)
6.  Information about you and your spouse’s previous marriages including dates and places of those marriages and the dates and places those marriages ended
7.  Information about the children who live with you, including their names, Social Security numbers, dates and places of birth
8.  Information about children not living with you, including their names, dates and places of birth, Social Security numbers, and amounts that you contribute in child support for them 
Copies of your marriage certificate and all divorce decrees (May be required in some cases)
Copies of the public birth records for each child you claim as a dependent (May be required in some cases)
Copies of the court records for adoption for each adopted child
9.  Information about your training and employment history for the past year, including
• name and address of employers
• beginning and ending dates of employment
10.  Information about your nursing home, if you live in one
11.  Information about your net worth and your dependents’ net worth
12.  Information about your recurring income and your dependents’ recurring income
13.  Information about income you and your dependents expect to receive in the next 12 months 
Current medical evidence telling us about your disabilities
If you are in a nursing home, attach a statement signed by an official of the nursing home that includes
• date you were admitted to a nursing home
• your level of care in the nursing home
Your nursing home payment status, which is Medicaid coverage or private pay

VOCATIONAL REHABILITATION AND EMPLOYMENT FOR SERVICE-DISABLED VETERANS
EVALUATION: If you have a VA combined service-connected disability rating of 10 percent or more and you apply for Vocational Rehabilitation and Employment, we will schedule you for a comprehensive

evaluation. As a first step during this evaluation interview, a counselor will work with you to answer three questions. For you to receive rehabilitation services and assistance under this program, the

answers to all three questions must be "yes."
1. Do you meet the basic entitlement requirements in the next paragraph?
2. If you meet these requirements, is it reasonable to expect you to be able to reach an employment or independent living goal?
3. Are you within the time limit for receiving this benefit? (This is generally 12 years from the date VA notified you that you had at least 10% service-connected disability.)
BASIC ENTITLEMENT REQUIREMENTS:

You are a veteran or a service member awaiting a disability discharge 20 percent disabling An employment handicap* Vocational Rehabilitation and Employment services and

assistance.
You are a veteran 10 percent disabling A serious employment handicap* Vocational Rehabilitation and Employment services and assistance.
   20 percent disabling A serious employment handicap* Independent living services and assistance.
* An employment handicap means that you have difficulty in getting or holding a job that is suitable for your aptitudes, interests, and abilities, and is compatible with your disabilities. Your difficulty in

getting and keeping a job must result in substantial part from your service-connected disability. A serious employment handicap means that this difficulty greatly limits your ability to get or hold a suitable

job.
PLANNING AND COUNSELING: Your counselor must first determine that you meet the entitlement requirements and an employment or independent living goal is reasonably feasible. Then your counselor

will help you develop a plan of services and assistance to assist you to reach your employment or independent living goal. Counseling will be available throughout your program to help you with problems

that may arise. VR&E will make every effort to set up evaluation and counseling meetings at times convenient to you.
REHABILITATION SERVICES: Not all Vocational Rehabilitation and Employment programs involve training. You may only need employment services to help you get a suitable job. If a VA counselor

determines that you need training to reach your vocational goal, your VA counselor will also determine the number of months of training you need. You may train in a vocational school, a special

rehabilitation facility, an apprenticeship program, other on-job training position, a college, or a university. While you train, VA will provide the medical and dental care and treatment, employment assistance

to get and keep a job, and other services you need. If a vocational goal is not currently feasible for you, VA may provide services and assistance to improve your capacity for independent living.
SUPPORT: VA may pay for tuition, fees, books, equipment, tools, or other supplies you need to  succeed in your program. While training and for two months after you complete training and are looking

for employment, you will receive a subsistence allowance to help you meet your living expenses. The allowance you receive depends on your type of training, rate of attendance, and number of

dependents. You will receive this allowance in addition to any VA compensation or military retired pay you may receive.
